Contract for the Minimum Support of the Temporary Comprehensive Hospital Software

This contract (“Contract”) is entered into on [Date] between [Company Name], a corporation organized under the laws of [State], with its principal place of business at [Address] (“Provider”), and [Client Name], a corporation organized under the laws of [State], with its principal place of business at [Address] (“Client”).

WHEREAS, Provider is in the business of developing and providing software solutions to healthcare organizations; and

WHEREAS, Client desires to engage Provider to provide minimum support for the Temporary Comprehensive Hospital Software (“TCHS”) developed by Provider.

NOW, THEREFORE, in consideration of the mutual covenants and agreements contained herein, the parties agree as follows:

1. Scope of Services

Provider shall provide minimum support for the TCHS to Client during the term of this Contract. Minimum support includes:

a. Technical support via email or phone during Provider’s regular business hours (Monday through Friday, 9:00am to 5:00pm Eastern Time).

b. Updates to the TCHS to fix bugs or other issues that may arise.

2. Term

The term of this Contract shall be [Number of Months] months, beginning on [Start Date] and ending on [End Date], unless terminated earlier in accordance with the provisions of this Contract.

3. Fees
